Checklist for your first appointment

• Your referral to one of our Specialists
• A list of any medications that you are taking
• Your Medicare Card/ Private Health Fund card
• Any relevant diagnostic imaging such as CT.s, MRI.s or OPG.s
• Any relevant reports from investigations such as pathology and audiograms
• Payment (we accept cash, EFTPOS and credit card payments, except American Express and Diners). We are not a bulk billing practice.
• New Patient Registration Form (Can also be completed on the day of your appointment)
